Our Physiotherapist has worked in the allied health sector running his own private practice for over 20 years. He has worked with thousands of patients with a history of chronic pain and has achieved effective results.

He has a passion for tailoring treatments to suit you in your home, has excellent patient rapport, is experienced with complex conditions and is driven to help you achieve your goals.

Our Physiotherapist is registered with The Australian Health Practitioner Regulation Agency (Ahpra). He is also a member of the Australian Physiotherapy Association (APA)

Chronic Disease Management (CDM) plan

A Chronic Disease Management (CDM) plan was previously known as an Enhanced Primary Care (EPC) plan.

What Medicare funded Physiotherapy rebates are available under a Chronic Disease Management (CDM) plan?

Medicare Australia offers Chronic Disease Management (CDM) plans to help people with chronic diseases manage their health and stay healthy. A CDM plan is coordinated by a health professional such as your GP and provides access to a range of services including allied health services such as physiotherapy. If you have a chronic disease, terminal condition, or complex care needs Medicare can provide a rebate for some of the costs of your treatment.

Am I eligible?

Children and Adults are eligible. Your GP will work with you to determine if you meet the eligibility criteria and then develop a GP Management Plan with Team Care Arrangements (TCA). The GP needs to determine that the patient has;

  • one or more chronic medical conditions that have been (or are likely to be) present for at least 6 months.
  • A terminal condition
  • complex care needs that require ongoing care from a multidisciplinary team consisting of their GP or medical practitioner, and at least 2 other health or care providers.

Do I need a referral?

Yes, a patient must be referred to Healthy Impact by their GP or medical practitioner. Your GP and their administration staff can organise this for you.

Do you provide a report back to the GP?

Yes, Under the CDM plan we must provide a written report back to the referring GP after the first and last service, or more often if necessary. Written reports include:

  • any investigations, tests, and/or assessments carried out on the patient:
  • any treatment provided; and
  • recommendations for future management of the patient’s condition or problem.

 

We work closely with several GPs in the area with patients on CDM plans. We are happy to liaise with new GPs outside our current referral network. You can simply give our details to your GP for Healthy Impact Mobile Physiotherapy to be a part of your care team.

What Medicare funded Physiotherapy rebates am I entitled to under a CDM?

Up to 5 Medicare funded Physiotherapy rebates of $58.30 per session each calendar year (as of Jan 2024). The number allied health sessions allocated for Physiotherapy funded sessions will be determined by your GP after discussing your needs with you.

What if I do not use all my Medicare funded Physiotherapy rebates in that calendar year?

You can simply use them next calendar year without the need for a new referral. However, these will count towards the maximum of 5 allied health services for the calendar year. At the end of your CDM plan your GP with discuss with you your progress and if appropriate develop a new plan for your ongoing care.

How do we charge for CDM plans?

Unfortunately, we are not able to offer bulk-billed appointments.

Physiotherapy sessions are paid for upfront, see our Physiotherapy fees. Patients are required to provide Medicare card details, date of birth, and the bank account they wish Medicare to process the CDM rebate too.

Due to privacy and security Medicare does not permit access to their systems, so we need your details to process the rebate for you. With your consent, we will process your Medicare funded Physiotherapy rebate electronically to your nominated bank account. This takes less than 24-48 hours. Once processed we will email you an invoice with the Medicare statement attached.  This makes the process quick and easy for you.

Can I claim on my Private Health as well?

No. You will need to decide if you use a Medicare CDM plan or your private health insurance extras cover to pay for these Physiotherapy services. Medicare states that “patients cannot use their private health insurance general treatment cover to ‘top up’ the Medicare rebate paid for the services.”

What if I am not eligible for a CDM plan?

Healthy Impact Mobile Physiotherapy also accepts private health fund rebates (via Medipass), DVA, NDIS, Workcover QLD, and Aged Care Package patients like Comlink.
